Display Mirroring Option icon won't show on Menu Bar despite activation through System Preferences

Upgraded to El Capitan 10.11.1 on my MacBook Pro (mid-2009 model; 2.53 GHz Intel Core 2 Duo) in early October and acquired the new Apple TV on its 2nd day of release 2 weeks ago. Love it. Am able to AirPlay from iPhone fine and Airplay audio from Safari...but it won't AirPlay video. For some reason, my Display Preference Menu Bar icon is refusing to display (when I click it off/on in System Preferences, it will momentarily flash appear and then disappear again). When in the Built-In Display System Preference window and hold down the Option button to "Detect Displays", nothing happens and I see no option to detect my Apple TV (hooked up to my Samsung Smart TV and sharing the same Internet/WiFi connections).


Question #1: How do I get my Monitor Display icon to remain visible on my Menu Bar (and why is it no longer remaining visible on its own)?


Question #2: How do I get my AirPlay to display video to my Apple TV (and why is only audio streaming through and not video as well)?
